The holster configuration that you're wearing, particularly if you have it strapped on tight, or leaning up against anything like a center console in the car, the pressure can engage the magazine release enough to actually disengage the magazine. Of course the magazine's not going to drop out in a vertical position while wearing it, but if you were to take the gun out of the holster the magazine could then fall to the ground and you may not hear it hit. It's possibly you laid it on a bed and when you picked it up, the loaded magazine slid out unnoticed... this is just a couple of my analyses. I wear my 45 in a similar type holster over my right hip pocket; leaning back in the seat, the magazine release will get enough pressure to release the magazine catch. I am now very aware of this and if I had to draw the gun, I will automatically slap the magazine back in just to make sure... you know ~ the old slap, rack and fire scenario. It could be very dangerous pulling a gun out and aiming at someone and having the magazine drop to the floor... HELLO! Not cool.
