I believe it is from Elkhart, Indiana. The 1916 - 1917 Dun & Bradstreet shows The New Brunswick B & B as being in Elkhart. Richards Token Database has it listed without an image. If you would like, you can send your image to Richards Token Database and they will use it with their description.
