Messed up nickle

My daughter got this nickle as change and don't know if someone tried to deface it or if it came that way from the mint. She was going to use it and I said no, so she gave it to me. I do believe those are Jefferson's eyes looking back...lol Thanks for looking. John
 
I agree, those are his eyes and nose! Is it defacing or mint error? Hard to tell from just a photo.

Looks like abrasion marks left from a Dremel like grinder, to me. But that's his eyes nose and mouth.
