Is this buckle real?

Yes, that would be a Civil War era box plate. Did you find it with any other items, like bullets or camp lead? Did you find it in a remote location? Because so many plates have been faked or repro'd and some of those lost, context is everything when it comes to knowing if what you found is good.
 
I've never seen a box plate with loops. I can't say for sure no, but box plates were brass and yours is iron.
 
I have just ran through the book "Confederate Belt Buckles & Plates" by Steve Mullinax and I can't find a box plate like yours. I can't find a regular CS oval like your. Most of the CS ovals that are close to yours don't have a filled back.
