Just wanted to let everyone know; I was one of the guys having the issue where I plugged in the 1/4" stereo to mono adapter, and then a smaller, inside the ear wrap around headphones and the speaker on the CTX was still on. It was intermittent. If I pushed sideways on the base of the adapter real hard, sometimes I could get it to turn off. I disassembled the headphone module and tinkered a bit. I stole this image from a similar post over at mlotv.

On the plug (center of the screen, where the colored wire is going into the portion that'd mate up to the headphone jack) the three wires are soldered to three silver contacts that touch the different portions of the headphone plug or adapter. Taking a flat bladed screwdriver, I gently, but firmly pushed on the backside of these. I wanted to make sure I was getting good contact at each point with my headphone jack.
Reassembled everything, and viola, no more intermittent speaker.
Just something to check out if you're having a similar issue.
