Headphone wiring help please

Can some please provide the wiring for the headphone?

I put together a plug adaptor for the CTX headphone which works great on the wireless module however when I plug directly into the machine I get no sound in the headphones, I'm using a stereo adaptor?
I'm sure its something simple but just can't figure it out?
